The practice is open to patients Monday – Friday, 8:00 am – 5:00 pm offering family medical care and mental health services. The Patient Centered Medical Home (PCMH) is a model for delivering comprehensive, coordinated, patient-centered primary care that is compassionate, accessible, and family-centered. The concept of PCMH is that improved access to primary care improves patient and provider experience, quality, and outcomes, as well as lowers cost.
