About the Practice
We work to keep individuals in the community in which they live by offering services that are evidence-based and specialized to meet the needs of serious mental illnesses. Assertive Community Treatment (ACT) is an evidence-based practice for individuals with serious mental illness who are most vulnerable to homelessness, incarceration and hospitalization. Staff work to provide short-term support for individuals transitioning to the community until they are linked to an ongoing treatment team.
