Companion & Compassion, LLC is a growing, nurse practitioner-led home health agency serving Jacksonville and Northeast Florida. As we expand our team and patient census, we are looking for a motivated Licensed Clinical Social Worker who is ready to make a real impact both clinically and strategically.

ABOUT THE ROLE

This is not a traditional social work desk job. We are looking for someone who thrives on autonomy, genuinely enjoys connecting with people in the community, and understands that strong referral relationships are the lifeblood of a growing home health agency. You will wear two important hats: clinical social worker serving our patients and families, and community ambassador building the agency's presence and referral network across Jacksonville.

This position begins part-time with hours expected to grow as our patient census and referral network expand. The right candidate will grow with us.

Overview

Clinical Duties:

• Review incoming patient referrals and identify social-service needs
• Complete psychosocial assessments and develop appropriate plans of care
• Provide counseling, education, crisis support, and community-resource coordination
• Assist patients and families with Medicaid, long-term care, transportation, housing, advance directives, and other available resources
• Collaborate with nurses, therapists, physicians, case managers, and caregivers
• Maintain timely, accurate, and compliant clinical documentation
• Participate in weekly meetings with the Administrator and provide reports on referrals, patient needs, completed activities, barriers, and follow-up items

Community & Marketing Duties:

• Build and nurture referral relationships with hospitals, rehabilitation facilities, physician offices, case managers, senior organizations, and other community partners
• Represent and market the agency professionally at community events, senior fairs, and networking opportunities
• Serve as a community liaison connecting patients to appropriate resources and positioning Companion & Compassion as the go-to home health provider in Jacksonville
• Identify opportunities to grow the agency's referral network and patient census through strategic outreach

QUALIFICATIONS

Required:

• Current, unrestricted Florida LCSW license
• Master's degree in Social Work (MSW) from an accredited program
• Minimum 1–2 years of social work experience
• Ability to work independently, prioritize responsibilities, and follow through without continuous supervision
• Strong communication, documentation, networking, and relationship-building skills
• Reliable transportation, valid Florida driver's license, and current automobile insurance
• Ability to travel throughout the Jacksonville service area
• Successful completion of Level 2 background screening

Preferred (Not Required):

• Home health, hospital, rehabilitation, skilled nursing, or community-based healthcare experience
• Familiarity with Florida Medicaid programs, Medicare home health, and senior services
• Experience with healthcare marketing, business development, or community outreach
• Bilingual in English and Spanish is a plus

Pay: $30.00 - $40.00 per hour

Expected hours: No more than 15.0 per week
