Mental Health Clinician – LPC, LMFT, LISW, LAC or Associate-Level Clinician

Hart to Heart Therapy, LLC is a growing private mental health practice committed to providing compassionate, ethical, and individualized care to children, adolescents, adults, couples, and families.

We believe that effective therapy begins with strong relationships—not only with our clients, but also within our clinical team. We are seeking two Mental Health Clinicians who are looking for an opportunity to grow professionally while becoming part of a supportive and collaborative practice.

Licensure

We are currently seeking clinicians who hold, or are working toward, one of the following South Carolina licenses:

• Licensed Professional Counselor (LPC)
• Licensed Marriage and Family Therapist (LMFT)
• Licensed Independent Social Worker (LISW)
• Licensed Addiction Counselor (LAC)
• Licensed Professional Counselor Associate (LPCA)
• Licensed Marriage and Family Therapist Associate (LMFTA)
• Licensed Addiction Counselor Associate (LACA)

Associate-level clinicians are encouraged to apply. Candidates should be committed to professional growth, ethical clinical practice, and providing high-quality care to clients.

Position Overview

The Mental Health Clinician provides outpatient behavioral health services based upon each client's presenting concerns, diagnosis, treatment goals, developmental needs, and individual circumstances.

Clinicians will conduct assessments, develop and maintain individualized treatment plans, provide psychotherapy within their areas of competence, coordinate care when appropriate, and maintain timely and accurate clinical documentation.

Primary Responsibilities

• Provide individual, couples, family, and/or child and adolescent therapy based upon training, experience, and scope of practice.
• Conduct comprehensive intake assessments and identify appropriate treatment needs.
• Develop individualized treatment plans in collaboration with clients.
• Maintain timely, accurate, and clinically appropriate progress notes, treatment plans, assessments, discharge documentation, and other required clinical records.
• Monitor client progress and modify treatment interventions when clinically indicated.
• Coordinate referrals and collaborate with other healthcare or community professionals when appropriate and authorized.
• Maintain professional and responsive communication with clients throughout the course of treatment.
• Maintain appropriate professional boundaries and adhere to ethical and legal standards governing clinical practice.
• Protect the confidentiality and privacy of all client information in accordance with HIPAA, applicable state and federal requirements, professional ethical standards, and Hart to Heart Therapy policies and procedures.
• Participate in supervision, consultation, training, and team collaboration as appropriate.
• Maintain professional licensure and complete required continuing education and professional development activities.
