Licensed Therapist – HBAI Assessment

Empowering Healthcare Services is seeking a qualified and compassionate Licensed Therapist to provide Health Behavior Assessment and Intervention (HBAI) services to geriatric patients in skilled nursing and long-term care settings.

The therapist will conduct behavioral health assessments, identify psychological and behavioral factors affecting patients’ overall health, and develop appropriate treatment recommendations in collaboration with the patient’s healthcare team.

Position: Licensed Therapist – HBAI

Employment Type: 1099 Independent Contractor

Setting: Skilled Nursing & Long-Term Care Facilities

Patient Population: Geriatric

Schedule: Flexible / Part-Time/Full-Time

Service Area: Metro Atlanta, Georgia

Required License

Only the following active Georgia licenses will be accepted

• LPC – Licensed Professional Counselor

• LCSW – Licensed Clinical Social Worker

No other license types will be accepted for this position.

Responsibilities

• Conduct comprehensive behavioral health and HBAI assessments.

• Evaluate psychological, behavioral, and social factors impacting a patient’s physical health and overall functioning.

• Identify behavioral health needs and provide appropriate clinical recommendations.

• Develop individualized treatment recommendations based on assessment findings.

• Provide brief behavioral interventions when clinically appropriate.

• Collaborate with physicians, nurse practitioners, nursing staff, facility staff, and other members of the healthcare team.

• Document assessments and services accurately and timely in the electronic medical record.

• Maintain compliance with applicable clinical, regulatory, and documentation requirements.

• Provide services consistent with geriatric behavioral health best practices.

• Communicate significant clinical concerns to the appropriate members of the treatment team.

Qualifications

• Active Georgia LPC or LCSW license.

• Master’s degree in Counseling, Social Work, or a related field appropriate to the qualifying license.

• Experience conducting behavioral health assessments preferred.

• Experience working with geriatric patients, skilled nursing facilities, long-term care, or medically complex populations preferred.

• Strong clinical assessment and documentation skills.

• Ability to work independently while collaborating effectively with facility and medical staff.

• Comfortable working within an electronic medical record system.

• Professional liability/malpractice insurance required.

• Must be comfortable working as a 1099 independent contractor.
