• Conduct thorough patient assessments to identify mental health needs, behavioral challenges, and social determinants affecting well-being
• Develop and implement individualized treatment plans utilizing evidence-based practices such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy techniques
• Provide clinical counseling for individuals and groups across various settings including outpatient clinics, inpatient units, assisted living facilities, and home/community care environments
• Facilitate crisis intervention and manage urgent situations with sensitivity and professionalism
• Document all patient interactions accurately within electronic medical records (EMR) systems while ensuring compliance with HIPAA regulations for patient privacy and confidentiality
• Coordinate case management efforts by collaborating with healthcare teams, families, and community resources to optimize patient outcomes
• Engage in clinical research activities related to behavioral health interventions or specific populations such as geriatrics, pediatrics, or individuals with developmental disabilities

• Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent in your state or region
• Proven experience working with diverse populations including children, seniors, individuals with disabilities, or those receiving hospice or post-acute care services
• Hands-on experience in child & family counseling, addiction counseling, dementia or Alzheimer's care, PTSD care, or working with individuals with autism or developmental disabilities is highly desirable
• Familiarity with hospital environments, outpatient clinics, nursing homes, assisted living facilities, or home & community care settings
• Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding for billing purposes, and medical documentation standards
• Proficiency with EMR systems and data collection methods to track patient progress effectively
• Strong skills in crisis management, diagnostic evaluation, mental health counseling, and group therapy techniques
