Licensed Social Worker (LSW)

Position Summary

Blue Point Nursing and Rehab Center is seeking a compassionate and dedicated Licensed Social Worker to join our interdisciplinary team. In this role, you will empower residents and their families by addressing psychosocial needs and ensuring the highest quality of care and quality of life.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop personalized care plans
• Provide emotional support and counseling to residents and families
• Coordinate seamless discharge planning and transitions of care
• Serve as a liaison between residents, families, and care teams
• Assist with admissions and resident adjustment
• Ensure compliance with state and federal regulations

Qualifications

• Licensed Social Worker (LSW/LCSW) in Maryland
• Experience in a skilled nursing facility preferred
• Strong communication and interpersonal skills
