Responsibilities
- Conduct thorough patient assessments to evaluate social, emotional, and environmental factors impacting health and well-being.
- Develop and implement individualized care plans aligned with clinical ethical guidelines and social work practice standards.
- Provide clinical counseling, including mental health counseling, behavioral therapy, and crisis intervention for diverse populations.
- Coordinate with healthcare teams to facilitate discharge planning, ensuring smooth transitions from inpatient to outpatient or community settings.
- Engage in case management activities such as data collection, documentation of medical records, and adherence to HIPAA regulations.
- Collaborate with community resources to connect patients with necessary services like hospice care, addiction counseling, or senior care programs.
- Lead group therapy sessions and utilize motivational interviewing techniques to support behavioral health goals.
- Maintain accurate medical documentation using EMR (Electronic Medical Records) systems and ensure compliance with ICD coding standards.
- Work with individuals from diverse cultural backgrounds and those with developmental disabilities or dementia to provide tailored support.
- Participate in clinical research initiatives related to social work practices or specific patient populations such as geriatrics or pediatrics.
Qualifications
- Valid license as a Licensed Social Worker (LSW or LCSW) in accordance with state regulations.
- Proven experience in hospital settings, inpatient care, outpatient clinics, or home & community care environments.
- Strong knowledge of medical terminology, CPT coding, ICD-9/10 coding systems, and Medicare requirements.
- Demonstrated expertise in child & family counseling, hospice & palliative medicine, and behavioral health interventions.
- Proficiency in crisis management, diagnostic evaluation, and working with individuals with autism or developmental disabilities.
- Excellent communication skills for client interaction and interprofessional collaboration.
- Ability to handle sensitive situations with empathy while adhering to HIPAA and clinical ethical guidelines.
- Experience working with geriatric patients, assisted living facilities, nursing homes, or post-acute care settings is highly desirable.
- Competency in using EMR systems and managing medical records efficiently.

Pay: $35.41 - $42.64 per hour
