Licensed Social Work Case Manager – Dementia Respite Care Program

Position: Licensed Social Work Case Manager

Position Summary

The Social Work Case Manager will provide case management, psychosocial support, supervision, and interdisciplinary coordination for the Dementia Respite Care Program. The Case Manager will help interns understand the social, emotional, and family-caregiving needs of individuals living with dementia.

Key Responsibilities

• Participate in orientation and program training and supervision.
• Provide guidance regarding communication, boundaries, caregiver needs, and person-centered support.
• Assist in understanding the impact of dementia on individuals and families.
• Help identify and appropriately communicate concerns related to social, emotional, environmental, or caregiver needs.
• Support appropriate referrals and coordination with community resources when needed.
• Collaborate with the RN, Direct Care Aide Trainer, and Program Coordinator.
• Participate in interdisciplinary meetings and case reviews.
• Monitor progress and provide constructive feedback.
• Support documentation, quality assurance, and program evaluation.
• Maintain confidentiality and comply with applicable privacy and organizational requirements.

Qualifications

• Master's level license LCSW or LICSW
• Experience working with older adults, caregivers, dementia, or community-based services preferred.
• Strong case management, communication, and supervisory skills.
• Commitment to culturally responsive, person-centered care.

Pay: $45.00 - $60.00 per hour

Expected hours: 7.0 – 12.0 per week

Work Location: Hybrid remote in Springfield, MA 01105
