Overview

Bebashi —Transition to Hope, a leading nonprofit organization serving Philadelphia's diverse and underserved communities, is seeking a compassionate, mission-driven Licensed Professional Counselor (LPC) to join our Wellness Center team. The LPC will provide high-quality behavioral health services as part of Bebashi's integrated care model, supporting clients' mental, emotional, and overall well-being through trauma-informed, culturally responsive counseling and care coordination.

Founded in 1985 as the first Black-founded and Black-led organization in the United States to address the HIV/AIDS crisis, Bebashi —Transition to Hope has grown into a comprehensive health and social services agency. Our mission is to empower people to enhance the quality of their health and overall well-being by providing access to highquality healthcare, health education, and social services.

Bebashi's Wellness Centers provide low-barrier, culturally competent primary care and preventive health services for uninsured, underinsured, and underserved individuals, with a particular focus on LGBTQ+ and BIPOC communities in Philadelphia.

Major Duties and Responsibilities

The Licensed Professional Counselor will be responsible for providing direct behavioral health services and supporting integrated care across Bebashi's Wellness Center programs. Responsibilities include:

Provide individual, group, and family counseling for clients referred through Bebashi's Wellness Center and case management programs.

Conduct behavioral health assessments and screenings, including PHQ -9, GAD-7 , ACEs, and other appropriate tools, to identify and address client mental health needs.

Develop, implement, and monitor individualized treatment plans in collaboration with clients, ensuring goals align with overall wellness, primary care, and social support needs.

Deliver trauma-informed, culturally responsive, and strengths-based therapy tailored to the needs of BIPOC, LGBTQ+, and other underserved populations. Provide crisis intervention, de-escalation, and safety planning for clients experiencing acute distress.

Collaborate with medical providers, case managers, prevention specialists, and other team members to ensure integrated, holistic care coordination.

Maintain detailed, compliant, and timely documentation in the Electronic Medical Record system.

Facilitate referrals to higher levels of care, including psychiatry, substance use treatment, and other specialized behavioral health services.

Participate in case conferences, quality improvement meetings, and multidisciplinary team huddles.

Provide staff education on behavioral health topics and contribute to community mental health outreach, as needed.

Experience and Qualifications

Education:

• Master's degree in Counseling, Psychology, Clinical Mental Health Counseling, or related field (required).

Licensure:

• Current, unrestricted Pennsylvania LPC license in good standing (required)

Experience:

• Minimum of three years of post-master's counseling experience in a clinical, community health, or nonprofit service setting.
• Demonstrated experience providing trauma-informed care to BIPOC, LGBTQ+, and other underserved communities.
• Familiarity with integrated primary care and behavioral health models preferred. Knowledge of Medicaid, Medicare, and other payer documentation standards strongly preferred.
• Proficiency with Microsoft Office Suite and Electronic Medical Record systems required; experience with eClinicalWorks preferred.

Preferred

• Bilingual (English/Spanish).
• Training or certification in trauma-informed care, substance use counseling, crisis intervention, or related clinical practice areas.

Working Conditions

• Primarily onsite at Bebashi's Primary Wellness Center (4101 Woodland Ave) with some travel to our North Philly Location (1217 Spring Garden)
• Monday—Friday, 9:00 AM—5:00 PM; occasional evenings or weekends for outreach or client needs.
• May include limited telehealth services as approved.
• Regular supervision and access to professional development resources provided.

Pay: $73,000.00 - $77,000.00 per year
