The Per Diem Clinical Social Worker is responsible for visiting patient homes throughout Rhode Island to provide them with evaluations and information on resources to allow them to remain at home safely while ensuring quality of life. The Per Diem Clinical Social Worker for Saint Elizabeth Home Care is a part of a supportive interdisciplinary team and they must communicate professionally in person and via telephone and email. The Per Diem Clinical Social Worker must be able to document findings and outcomes via company software and is outfitted with an iPad.
Qualifications for a Clinical Social Worker
- Licensed Clinical Social Worker in the state of Rhode Island.
