Licensed Clinical Social Worker

We have an opening for an LCSW in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. The buildings we need covered for this position include Independence Village Fishers South (NE), Crown Point of Indianapolis (E), Worthington Place (SW) and Hellenic of Indianapolis (S). We travel to 1 building per day, but all of our buildings weekly.

Our focus is primary care and psychiatric care in assisted living communities. We are offering a full time position to an LCSW to better serve our clients medical needs.

Position Overview: We are seeking a dedicated and compassionate Licensed Clinical

Social Worker (LCSW) to join our multidisciplinary team. This role involves providing high quality psychotherapy services alongside a Psychiatric Nurse Practitioner (NP) and managing chronic care coordination to enhance patient outcomes. The LCSW will play a crucial role in delivering comprehensive mental health support and coordinating care for patients with chronic conditions. We are looking for a provider to cover multiple Assisted Living facilities in the Columbus, New Whiteland and Greenfield. We travel to 1 facility/day, but multiple facilities weekly.

Due to the unique nature of the position, the hiring process will require a 2 hour shadow with one of our current providers in the Indianapolis area to ensure the position is a good fit for both parties.

Starting pay is around $80k but with our incentive based structure (based on psychotherapy coding and reimbursement from CMS), our current LCSWs are averaging closer to $100k.

Key Responsibilities:

Psychotherapy:

• Provide individual therapy to clients, employing evidence-based practices and

therapeutic modalities.

• Conduct thorough assessments of patients' mental health, including diagnoses and

treatment planning.

• Develop and implement treatment plans in collaboration with patients, their

families, and the Psychiatric NP.

• Monitor patient progress and adjust treatment plans as necessary.
• Maintain accurate and confidential records of therapy sessions and treatment

outcomes.

Chronic Care Management:

• Work closely with the clinical team to develop and implement care plans for

patients with chronic conditions.

• Assist in the coordination of care between various providers, ensuring a holistic

approach to patient health.

• Facilitate patient education and self-management strategies for chronic illness.
• Address psychosocial factors affecting patients’ health and treatment adherence.
• Advocate for patients’ needs and assist in navigating healthcare services and

resources.

Collaboration:

• Collaborate with the Psychiatric NP to integrate mental health care with overall

patient treatment plans.

• Participate in case meetings and care coordination discussions to ensure

comprehensive patient management.

• Provide insights and recommendations based on psychosocial assessments and

observations.

Documentation and Compliance:

• Maintain accurate and timely documentation of all patient interactions and

treatment plans.

• Ensure compliance with all relevant regulations, ethical standards, and

organizational policies
