About the role
Looking for a Licensed Clinical Social Worker needed for palliative care services to do home visits for the South Los Angeles, South Bay, and Long Beach area.
Must have Home Health and or Hospice/Palliative Care experience.
Must be Licensed and Credential as a Clinical Social Worker for this position
Duties: Advocating for and develop plans to improve patients well-being. Research and refer patients to community based resources. Help patients with IHSS paperwork, Advanced Directives/etc.

Works as a independent contractor
Required to do live in-home visits
Covid Results or Vaccination required
Current TB/Chest x-ray required
Flu Vaccine required
Physical Exam required
Current CPR required
Driver License required
Car Insurance required
