Job Title: Licensed Clinical Social Worker (LCSW)

Location: Illinois (travel to patients’ residences required)

Position Type: Full-time and part-time positions available

Experience Level: 0-3 years as a social worker

Responsibilities

• Provide direct clinical services to patients, including assessment, diagnosis, treatment planning, and counseling.
• Conduct psychosocial assessments and develop individualized care plans.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Travel to patients’ homes for assessments, follow-up visits, and crisis intervention.
• Maintain an active LCSW license in the state of Illinois.
• Adhere to ethical and legal standards in social work practice.
• Participate in case conferences and team meetings.
• Document patient interactions accurately and timely.
• Assist patients in accessing community resources and support services.
• Provide crisis intervention and support during emergencies.
• Engage in ongoing professional development and training.
• Psych therapy
• patient evaluation

Qualifications

THIS IS A PAY PER VISIT JOB.

• Master’s degree in Social Work (MSW) from an accredited program.
• Active LCSW license in the state of Illinois.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate, empathetic, and culturally sensitive.
• Proficient in electronic health records (EHR) systems. HOSPICE MD, AXXCESS
• Knowledge of relevant laws, regulations, and ethical guidelines.
• Experience working with diverse populations and addressing mental health needs.
• Willingness to travel to patients’ residences.

Compensation

• Negotiable based on experience and skill level.
