Licensed Clinical Social Worker (LCSW) / Licensed Professional Counselor (LPC)

Only licensed clinicians will be considered, as this role requires contracting with Medicare.

Overview

We are seeking a New Jersey-licensed LCSW or LPC to provide individual therapy and psychotherapy to residents of senior living communities. This is a full-time hybrid position with a consistent Monday–Friday daytime schedule, offering stability and the opportunity to build meaningful therapeutic relationships with aging adults.

Ability to provide in selected days onsite in-person therapy/psychotherapy in Hammonton, NJ and / or in Wayne is preferred.

Duties

• Provide individual psychotherapy sessions to older adults in assisted living and memory care settings.
• Conduct initial assessments, create treatment plans, and document progress in compliance with clinical and regulatory guidelines.
• Collaborate with facility staff, care coordinators, and family members to ensure comprehensive and coordinated care.
• Support residents with a range of mental health needs including depression, anxiety, grief, trauma, cognitive decline, and adjustment issues.
• Utilize geriatric-appropriate interventions, including CBT, reminiscence therapy, and supportive counseling.
• Participate in interdisciplinary meetings and clinical case reviews as needed.
• Maintain timely and accurate documentation in the electronic health record (EHR) system.
• Uphold ethical and professional standards in line with state licensure and organizational policies.

Qualifications

• Required Licensure: LCSW, LPC, LMFT, or equivalent (independent license required).
• Preferred Experience (not required): Minimum of 1 year providing therapy to older adults or in long-term care settings.
• Strong communication and rapport-building skills with older adults and facility staff.
• Competency in documentation and EHR systems.
• Ability to work independently in a field-based setting with minimal supervision.
• Compassionate, patient, and adaptable in working with aging and cognitively impaired populations.
