Serenity Care Health Group (SCHG) is seeking a compassionate LCSW or LMFT to join our growing integrated behavioral health team within our FQHC setting. You will work alongside primary care providers and support staff to provide accessible behavioral health services to diverse patient populations and help address barriers to care through a whole-person approach.

What You'll Do

• Provide outpatient behavioral health services, including individual, family, and group therapy for patients with a range of behavioral health needs.
• Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
• Utilize evidence-based interventions with an emphasis on trauma-informed and culturally responsive care.
• Collaborate with medical providers through warm hand-offs, consultations, and interdisciplinary care.
• Provide crisis intervention and stabilization when clinically appropriate.
• Maintain accurate and timely documentation in the Electronic Health Record (EHR) in accordance with FQHC, state, and federal requirements.
• Participate in interdisciplinary case reviews, staff meetings, and professional development activities.

What We're Looking For

• Active California LCSW or LMFT license in good standing.
• Master's degree in Social Work, Marriage and Family Therapy, or a related field.
• Recent graduates who have obtained their independent LCSW or LMFT license are encouraged to apply.
• Prior experience in community health, an FQHC, integrated care, or serving underserved populations is preferred, but not required.
• Experience with trauma-informed behavioral health services is valued.
• Bilingual Spanish and/or Khmer skills are highly valued, but not required.
• Strong clinical judgment and the ability to work effectively both independently and as part of a collaborative care team.
• Experience or a clinical background working with individuals with substance use disorders (SUD) is highly valued, but not required.
