About the role
Hometown Health Clinic is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team. The LCSW will provide outpatient behavioral health services, including assessment, diagnosis, counseling, treatment planning, and crisis intervention, while collaborating with primary care providers and other members of the healthcare team.
Duties and Responsibilities
- Conduct behavioral health assessments and diagnostic evaluations.
- Develop and implement individualized, evidence-based treatment plans.
- Provide individual and family counseling.
- Monitor patient progress and modify treatment plans as appropriate.
- Assess behavioral health crises and coordinate appropriate interventions, referrals, or higher levels of care.
- Collaborate with healthcare providers to support coordinated, patient-centered care.
- Connect patients and families with appropriate community resources.
- Maintain timely and accurate documentation in the electronic health record.
- Comply with HIPAA and all professional, ethical, payer, and organizational requirements.
Qualifications
- Master’s degree in social work from an accredited program.
- Current, unrestricted Licensed Clinical Social Worker license in Tennessee.
- Experience in behavioral health assessment, diagnosis, counseling, treatment planning, and crisis intervention.
- Strong clinical judgment, communication, and documentation skills.
- Ability to work independently and collaboratively with a multidisciplinary healthcare team.
- Must be eligible to enroll and remain credentialed with Medicare, TennCare, and other participating insurance plans.
- Experience in an outpatient clinic, primary care, or integrated behavioral health setting is preferred.
