Licensed Clinical Social Worker (LCSW)

DocCare is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our multidisciplinary team. The LCSW will provide therapeutic services, and support patients with complex needs. This is a part-time position working up to 20 hours per week. This position is for our location in Hauppauge, NY. The ideal candidate will have a strong commitment to patient advocacy, clinical excellence, and a holistic approach to health. Responsibilities

• Conduct mental health assessments to evaluate patients’ mental health, social, and environmental needs.

• Develop, implement, and monitor individualized treatment plans in collaboration with patients, families, and healthcare providers.

• Provide evidence-based therapy, including individual, group, and family counseling.

• Coordinate care with other healthcare professionals, including primary care providers, psychiatrists, and community resources.

• Educate patients and their families on mental health conditions, coping strategies, and available resources.

• Maintain accurate and timely clinical documentation in compliance with company policies and state regulations.

• Participate in case consultations, team meetings, and training sessions to support collaborative care efforts.

• Advocate for patients and connect them with community services, such as housing, financial aid, or support groups, as needed.

• Other related duties as assigned.

Education

Master's degree in social work (MSW) required. Minimum 2 years of experience in a healthcare or behavioral health setting.

Minimum Requirements

Licensed clinical social worker (LCSW) in the state of practice required. Must have proficiency in evidence-based therapeutic modalities (e.g., CBT, DBT, trauma-informed care, etc.). Strong interpersonal and communication skills required. Must have the ability to work independently and as part of a multidisciplinary team. Experience working with diverse populations and addressing health disparities needed. Must have experience working with specific populations, such as children, adolescents, adults, families, or older adults.

Preferred Qualifications

5 years of experience strongly preferred. Familiarity with electronic medical records (EMR) systems and telehealth platforms is a plus. Bi-Lingual strongly preferred.

Job Type: Part-time

Pay: $44,000.00 - $52,000.00 per year
