Coastal Connecticut Counseling is expanding access to mental health care for older adults and Medicare clients, including in-home services for individuals who may experience medical, mobility, transportation, or other barriers to traditional office-based care.

We are looking for a seasoned Licensed Clinical Social Worker with specialized training and experience supporting older adults through grief and loss, end-of-life concerns, chronic or complex medical conditions, anxiety, depression, changes in independence, caregiver stress, and the many transitions that can accompany aging.

This role offers a high level of flexibility and autonomy. You will manage your schedule, maintain direct communication with your clients, and take ownership of the day-to-day organization of your caseload. Because of that independence, we are looking for someone who is highly organized, responsive, detail-oriented, and consistent with documentation and follow-through.

At Coastal, you will have the independence to manage your clinical work while remaining part of an established and collaborative practice. Coastal provides referrals, billing, credentialing support, an electronic health record, administrative support, and clinical consultation, allowing you to focus on providing exceptional care and developing meaningful relationships within the communities we serve.

Position Expectations

• Maintain an active caseload of at least 12–15 billable client hours per week.
• Provide outpatient and in-home psychotherapy to older adults and Medicare clients throughout the Fairfield and Westport area.
• Independently manage client scheduling and maintain timely, professional communication with clients.
• Complete assessments, treatment plans, progress notes, and required clinical documentation accurately and within established timeframes.
• Maintain strong organization, attention to detail, and follow-through across all aspects of your clinical responsibilities.
• Provide evidence-informed care within your areas of demonstrated training and clinical competence.
• Support clients navigating grief and loss, end-of-life concerns, chronic or complex medical conditions, anxiety, depression, changes in independence, caregiver stress, and other transitions associated with aging.
• Collaborate with family members and caregivers when clinically appropriate and with client consent.
• Coordinate care with physicians, medical providers, rehabilitation programs, senior services, and other community supports when clinically appropriate.
• Recognize when a client's needs extend beyond outpatient care and assist with appropriate resources, referrals, or higher levels of care.
• Maintain appropriate professional, ethical, and clinical boundaries when providing services in clients' homes and community settings.
• Practice independently while utilizing clinical consultation appropriately for complex clinical situations.
• Provide ethical, compassionate, reliable, and high-quality clinical care.

Compensation

• $60 per billable client hour
• Flexible, part-time schedule.
• Minimum of 12–15 billable client hours per week.
• Mileage reimbursed for approved client travel at the applicable reimbursement rate.
• Reimbursable mileage is calculated from the designated Coastal office to the client's service location, not from the clinician's home.

Qualifications

We are looking for a clinician who has intentionally developed the experience, training, and clinical skills necessary to support older adults and who is comfortable working with a high degree of professional autonomy.

Requirements

• Master's degree in Social Work.
• Active independent Connecticut licensure as an LCSW in good standing.
• Minimum of 5 years of post-graduate clinical experience.
• Demonstrated clinical experience working with older adults and/or Medicare populations.
• Proof of relevant certifications, continuing education, and/or specialized training related to the needs of this population, such as aging, grief and bereavement, end-of-life care, chronic or complex medical conditions, caregiver support, or related areas.
• Strong clinical judgment and demonstrated ability to practice independently.
• Exceptional organization, time management, attention to detail, communication, and follow-through.
• Ability to independently manage scheduling, client communication, documentation, and caseload responsibilities.
• Strong commitment to ethical practice, professionalism, and client-centered care.
• Valid driver's license, reliable transportation, and willingness to travel throughout the Fairfield and Westport service area.

Clinicians who are already credentialed and paneled with Medicare are strongly preferred.
