Licensed Clinical Social Worker

Summary

The Licensed Clinical Social Worker (LCSW) is responsible for providing interventions to remove psychosocial barriers preventing safe and timely transition through the continuum of care for adults and children. Skillfully advocates in empowering patients to maintain a level of control over their treatment and for self-determination.

Primary Essential Duties

• Instructs, treats, observes and evaluates patients exhibiting significant social and emotional problems affecting their health status.

• Assists the physician and other health team members in understanding the significant social and emotional factors related to the patient’s health problems.

• Participates in the development and periodic re-evaluation of the physicians Plan of Treatment for patients needing social work services.

• Utilizes special skills and judgment to resolve social and emotional responses connected with the illness.

• Observes, records and reports changes in patients' condition and response to treatment to Clinical Manager and the physician.

• Participates in the discharge planning process.

• Instructs/counsels patients and families in treating and coping with social and emotional response connected with illnesses.

• Provides ongoing assessment of patient and family needs and responses to teaching.

• Participates in care coordination activities and acts as a resource to other health team members in the identification and resolution of patient needs.

• Utilizes appropriate community resources to achieve identified objectives.

Skills and Qualifications

• Strong oral communication skills required to effectively communicate with a broad range of individuals.

• Strong written communications skills required to maintain quality documentation.

• Ability to prioritize all aspects of the job is a key to success.

• Ability to provide group, individual, and family counseling sessions.

• Strong problem solving skills required and must be a common sense thinker who can be a quick decision maker when necessary.

• Must display professionalism and leadership qualities.

• Knowledge and respect of all confidentiality issues.

Supervisory Relationships

• Reports to: Manager of Behavioral Health

• Supervision-Others: n/a

Education, Professional Training & Experience

• Master’s Degree in Social Work, Psychology, Counseling or other related field required.

• Actively Licensed in state of California as a Clinical Social Worker (LCSW); preferably 2 years minimum.

Working Conditions

The physical demands described here are representative of those required for the position. Position requires sitting, walking, standing, bending, and twisting in the performance of daily activities. The position requires hand manipulation and repetitive hand movement and fine coordination in using a computer keyboard. The position requires near and far vision in reading reports and use of a computer. Acute hearing is required in supporting meetings and providing phone and in‐person customer service. The position occasionally requires lifting and/or moving objects up to 20 pounds. Pay: $115,000.00 - $120,000.00 per year
