Job Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker/Clinical Social Worker to join our dynamic healthcare team. Duties

• Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, utilizing clinical terminology and adhering to confidentiality policies such as HIPAA.
• Develop and implement individualized treatment plans incorporating therapies like cognitive behavioral therapy, psychodynamic therapy, behavioral therapy, and evidence-based practices tailored to each patient’s needs.
• Provide individual therapy, group therapy, family counseling, and crisis intervention to support patients with diverse conditions such as PTSD, addiction, autism spectrum disorder, developmental disabilities, Alzheimer’s care, and geriatrics.
• Coordinate care through case management activities, including intake procedures, medical social work documentation, and collaboration with multidisciplinary teams to ensure holistic patient care.
• Maintain accurate clinical documentation in electronic medical records (EMR/EHR systems), ensuring compliance with medical documentation standards and ICD coding for diagnoses like ICD-9/10.
• Participate in ongoing professional development related to medical terminology, psychopharmacology, special education strategies for pediatrics and autism spectrum disorders, and crisis management protocols.

Skills

• Proven expertise in psychotherapy modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, applied behavior analysis (ABA), and addiction counseling.
• Strong experience working with children, adolescents, individuals with developmental disabilities or autism spectrum disorder (ASD), and adults.
• Proficiency with EMR/EHR systems for documentation of patient interactions while maintaining strict adherence to HIPAA regulations.
• Excellent patient assessment skills combined with the ability to develop effective treatment plans aligned with clinical confidentiality policies.
• Knowledge of ICD coding systems (ICD-9/10) for accurate diagnosis documentation; familiarity with CPT coding is a plus.
• Ability to manage crisis situations effectively through crisis management techniques and trauma-informed care approaches.
• Experience working within hospital settings, home & community care environments is highly desirable for delivering comprehensive patient-centered services.
• Clinical supervision is provided if needed.
