Job Overview

SNF Behavioral is a growing behavioral health organization providing psychosocial therapy and behavioral health services to residents in skilled nursing and long-term care facilities.

We are currently seeking LCSWs, LCPCs, and LMFTs to join our team in the Coles County / Mattoon, IL area.

This is an excellent opportunity for both experienced clinicians and newly licensed professionals looking to build their clinical experience in a supportive, professional environment with a consistent workflow and ongoing caseload.

The Opportunity

As a Behavioral Health Clinician, you will provide direct behavioral health and psychosocial therapy services to residents in SNF and LTC settings.

You will work closely with residents, facility staff, physicians, nursing teams, social services, and other healthcare professionals to identify behavioral health needs and provide ongoing therapeutic support.

Responsibilities

• Conduct psychosocial and behavioral health assessments
• Provide individual, family, and/or group psychotherapy as appropriate
• Develop and maintain individualized treatment plans
• Provide ongoing talk therapy and supportive counseling
• Address a variety of behavioral health and psychosocial needs, including adjustment, anxiety, depression, grief, behavioral concerns, and other emotional challenges
• Monitor resident progress and update treatment plans as needed
• Participate in resident care planning and interdisciplinary meetings
• Collaborate with nursing, social services, physicians, and facility leadership
• Communicate effectively with the interdisciplinary care team
• Maintain timely and accurate clinical documentation
• Follow all applicable professional, ethical, and regulatory requirements

Qualifications

• Active Illinois LCSW, LCPC, or LMFT license
• Master's degree in Social Work, Counseling, Marriage & Family Therapy, or related field
• Ability to provide clinical services within the scope of your license
• Strong communication and clinical documentation skills
• Professionalism and commitment to quality patient care

Full-time | Monday–Friday
