Hospice Social Worker (MSW/LMSW/LCSW)

Full-Time \| $35.00–$40.00 per Hour \| Weekly Pay

Seasons Care Hospice is seeking a compassionate and dedicated Hospice Social Worker to join our growing team. In this role, you will provide emotional support, counseling, care planning, and community resource coordination for hospice patients and their families.

This position will provide coverage for patients in Gainesville, Ellijay, and Cumming, GA. Reliable transportation is required, as travel between assigned locations is part of the role.

Responsibilities

• Provide psychosocial assessments for hospice patients and their families.
• Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team.
• Offer emotional support, counseling, grief support, and crisis intervention.
• Assist patients and families with community resources, financial assistance programs, and advance care planning.
• Participate in interdisciplinary team meetings and care conferences.
• Maintain accurate and timely documentation in accordance with hospice regulations.
• Educate patients and families regarding hospice services and available resources.
• Ensure compliance with all federal, state, and agency policies and regulations.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program required.
• Georgia Social Work license (LMSW or LCSW preferred).
• Previous hospice, home health, hospital, or healthcare social work experience preferred.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Valid driver's license, reliable transportation, and proof of auto insurance required.
• Ability to travel between Gainesville, Ellijay, and Cumming as needed.
