Coney Island Homecare is looking for a compassionate and dedicated Hospice Social Worker (LCSW) to join our team in beautiful Bozeman, MT. In this meaningful role, you'll provide essential emotional, psychosocial, and practical support to patients and their families during life's most profound moments. Responsibilities

• Conduct comprehensive psychosocial assessments for hospice patients and their families
• Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
• Provide counseling and emotional support to patients and families navigating end-of-life transitions
• Assist with advance care planning, including completion of advance directives and goals-of-care conversations
• Connect patients and families with community resources and financial assistance
• Provides and coordinates bereavement services for families and the community
• Participate in regular interdisciplinary team meetings to coordinate and communicate patient care
• Maintain accurate and timely clinical documentation in compliance with hospice regulations

Requirements

• Master's degree in Social Work (MSW) from an accredited program required
• Active Licensed Clinical Social Worker (LCSW) licensure in the state of Montana
• Prior experience in hospice, palliative care, or end-of-life care strongly preferred
• Strong knowledge of psychosocial assessment, grief counseling, and crisis intervention
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently in a home-based or community care setting
• Valid driver's license and reliable transportation for patient home visits
