What You'll Do
As a Licensed Clinical Hospice Social Worker, you will support patients and families as they navigate the emotional, social, financial, and practical challenges associated with serious illness and end-of-life care.
You will:
- Advocate for patients and their families and ensure their needs are heard and addressed
- Help patients and families navigate end-of-life planning and care decisions
- Provide emotional support and guidance during times of crisis and transition
- Help patients and families manage the emotional, familial, and financial stress associated with debilitating illness
- Connect patients and families with appropriate community resources and support services
- Collaborate with the Interdisciplinary Group (IDT) to develop individualized plans of care
- Communicate effectively with patients, families, caregivers, skilled nursing facility staff, hospital staff, and other healthcare providers
- Help identify barriers to care and develop creative solutions to meet patient and family needs
- Document accurately, thoroughly, and in a timely manner
- Work collaboratively with the care team to provide the most effective and compassionate care possible

- Current licensure and qualifications required for the position

Pay: $30.00 - $50.00 per hour
