Overview
Join our compassionate Modern Hospice team as a Hospice Social Worker, LCSW (Licensed Clinical Social Worker) and make a meaningful difference in the lives of patients and their families during their most vulnerable moments. Duties
- Conduct comprehensive patient assessments to evaluate psychosocial needs, emotional well-being, and family dynamics within hospice care settings.
- Provide individual, family, and group counseling utilizing clinical techniques such as cognitive behavioral therapy, motivational interviewing, and crisis intervention to support patients and families coping with terminal illness.
- Coordinate community resources including support groups, home health services, legal assistance, and spiritual care to ensure holistic patient support.
- Assist with discharge planning by collaborating with medical teams to develop personalized care plans that respect patient wishes and clinical guidelines.
- Maintain accurate medical documentation in electronic health record (EHR) systems, ensuring compliance with HIPAA regulations and social work practice standards.
- Educate patients and families about end-of-life options, hospice services, advanced directives, and available community programs.
- Participate in interdisciplinary team meetings to contribute insights on psychosocial aspects of patient care and support clinical decision-making.
Experience
- Licensed as a Clinical Social Worker (LCSW) with current certification in hospice & palliative medicine or related fields preferred.
- Demonstrated experience working with geriatric patients, including dementia care and Alzheimer's disease management.
- Proven ability to provide child & family counseling, behavioral health interventions, and crisis management within healthcare or community settings.
- Familiarity with medical terminology, ICD coding (ICD-10), CPT coding, Medicare regulations, and medical records documentation requirements.
- Experience working in hospice care environments or inpatient settings such as hospitals or nursing homes is highly desirable.
- Strong skills in community resource coordination, discharge planning, case management, and working with diverse cultural backgrounds.
- Knowledge of clinical research practices related to hospice care or mental health counseling is a plus.

Pay: From $60,000.00 per year
