Job Overview

We’re seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. In this role, you’ll provide emotional support, resource coordination, and advocacy for patients and families navigating end-of-life care. You’ll be a vital part of ensuring dignity, comfort, and peace during life’s most tender moments

Responsibilities

• Conduct psychosocial assessments for hospice patients and families
• Develop individualized care plans in collaboration with the hospice team
• Provide grief counseling, crisis intervention, and emotional support
• Assist families with advance directives, financial concerns, and community resources
• Facilitate family meetings and end-of-life planning discussions
• Maintain accurate documentation in compliance with Medicare and agency standards
• Participate in interdisciplinary team meetings and contribute to holistic care planning

Qualifications

• Master’s degree in Social Work (MSW) from an accredited program
• Current state licensure (LMSW or LCSW preferred)
• Minimum 1 year of experience in hospice, palliative care, or medical social work
• Strong communication, empathy, and organizational skills
• Valid driver’s license and reliable transportation

Job Type: Part-time

Pay: $33.00 - $36.62 per hour
