K&N Rehab Inc. is a trusted and growing home health company serving the Greater Los Angeles area. We are seeking compassionate and skilled Physical Therapists (PTs), Physical Therapist Assistants (PTAs), Occupational Therapists (OTs), Certified Occupational Therapy Assistants (COTAs), Speech Therapists (STs), and Licensed Clinical Social Workers (LCSWs) to join our interdisciplinary home health team.

Coverage Areas

We serve patients throughout San Diego County, including but not limited to:

• San Diego
• Coronado
• La Mesa
• Chula Vista

We offer flexibility to choose your preferred coverage area.

Key Responsibilities

Physical Therapists (PT) & Physical Therapist Assistants (PTA)

• Evaluate or assist in evaluating patients’ physical needs and abilities.
• Develop and implement treatment plans to improve mobility, strength, and function.
• Provide therapeutic exercises, gait training, and mobility education.
• Educate patients and caregivers on safety and at-home programs.
• Maintain accurate, timely documentation and collaborate with care teams.

Occupational Therapists (OT) & Certified Occupational Therapy Assistants (COTA)

• Evaluate or assist in evaluating patients’ functional abilities and rehabilitation needs.
• Develop or carry out treatment plans to enhance independence in activities of daily living.
• Provide adaptive techniques, equipment training, and caregiver instruction.
• Monitor progress, update care plans, and maintain documentation compliance.

Speech Therapists (ST)

• Evaluate and treat patients with communication, cognitive, and swallowing disorders.
• Develop individualized treatment plans and implement therapy to improve speech clarity and language skills.
• Educate patients and families on communication strategies and home exercises.
• Track progress and maintain thorough clinical documentation.

Licensed Clinical Social Workers (LCSW)

• Conduct psychosocial assessments and provide counseling to patients and families.
• Develop care plans addressing emotional, social, and community resource needs.
• Assist patients in accessing services, benefits, and financial support.
• Collaborate with healthcare teams for coordinated, holistic care.
• Provide crisis intervention and accurate case documentation.

Requirements (All Disciplines)

• Active and valid California license (PT, PTA, OT, COTA, SLP, or LCSW) in good standing
• EIN (Employer Identification Number) or willingness to obtain one
• Valid CA Driver’s License and reliable transportation
• Auto insurance
• Current CPR Certification
• Up-to-date Physical Exam & TB test
• Excellent communication, documentation, and organizational skills
• Ability to work independently and manage a flexible schedule
• Home health experience preferred, but not required
