Job Description

Wheatfield Pediatrics is seeking a compassionate and motivated HealthySteps Specialist (LMSW) to join our pediatric primary care team.

HealthySteps is an evidence-based program that promotes nurturing caregiving and supports the healthy development and well-being of babies, toddlers, and young children ages 0–5. The HealthySteps Specialist works directly within the pediatric office, partnering with families and medical providers to address developmental, behavioral, social, and emotional needs during the critical early years of a child's life.

This is a unique opportunity for a social worker who enjoys building relationships with young children and their families while working as an integrated member of the pediatric healthcare team.

Duties and Responsibilities

• Promote and support the HealthySteps program through team-based pediatric care focused on child development, behavior, relational health, and family well-being.
• Participate in well-child visits and provide support, education, and guidance to parents and caregivers of children birth through age 5.
• Provide short-term consultations for common early childhood concerns, including development, behavior, parenting challenges, routines, sleep, feeding, and family stressors.
• Help families understand developmental milestones and provide strategies that encourage healthy development and positive parent-child relationships.
• Identify family needs and connect families with appropriate community resources and services, including Early Intervention and other developmental, behavioral, and social supports.
• Maintain and manage referral resources, track follow-ups, and help ensure families successfully connect with recommended services.
• Collaborate closely with pediatric primary care providers and other members of the maternal-child and healthcare teams to provide coordinated, family-centered care.
• Provide ongoing follow-up and support to families when developmental, behavioral, emotional, or social concerns are identified.
• Participate in ongoing HealthySteps training, consultation, and supervision.
• Document patient and family interactions, referrals, follow-up, and other clinical activities accurately in the EMR.

Required Qualifications

• Master’s degree in Social Work (MSW) with LMSW licensure or eligibility for appropriate New York State licensure
• Strong interest in working with infants, young children, and their families
• Knowledge of early childhood development, family dynamics, and common developmental and behavioral concerns
• Excellent oral and written communication skills
• Empathetic, supportive, patient, and family-centered approach
• Ability to develop trusting relationships with parents and caregivers
• Ability to work effectively both independently and as part of a multidisciplinary pediatric team
• Strong organizational, communication, and follow-up skills
• Ability to work with families from a variety of backgrounds and cultures
• Experience in pediatrics, early childhood development, behavioral health, maternal-child health, or community-based services is preferred

Rather than waiting until concerns become larger problems, you will work alongside pediatric providers to identify needs early, provide families with practical support, and connect them with resources that can make a lasting difference.

You will become part of a collaborative pediatric team where developmental, behavioral, emotional, and social health are viewed as important parts of a child's overall health.

Schedule: Full-Time, Monday–Friday

Salary: $55,000–$60,000 annually, based on experience
