Monday-Friday 8:00am-4:00pm
LIAD History: Founded in Port Washington in 1988, the LIAD Center provides resources, programs, and support from pre-diagnosis through end of life for individuals with Alzheimer’s disease and their families, throughout Long Island. The mission of LIAD is to improve the quality of life of those living with Alzheimer’s disease and related forms of dementia and their families.
Responsibilities and Duties
Director of Social Adult Day Program Services
- Supervision of Social Adult Day Program including the early, moderate, and late-stage programming, including participants
- Supervision of day program staff including Day Program Coordinator, program assistants, bus drivers, student interns and volunteers.
a) Leading daily morning meetings and monthly check-ins regarding day program, approving time off requests, coordinating staff schedules and day program coverage.
b) Manage and assess prospective job applicants and make recommendations regarding recruitment/hiring of new staff.
- Maintaining confidential participant records and creating care plans, writing quarterly notes and reviewing and completing the annual assessments with MLTC’s-Aetna, VNY and Senior Whole Health, as required by NYS DOH.
- Ongoing clinical assessments of participants in program including, but not limited to, their progress adjusting to the routine and activities, transitions from one stage to another, dementia-related symptoms, and behaviors, their needs and safety in the program, and day program discharges.
- Communicating daily, weekly, and monthly with family caregivers to discuss clinical observations of participant needs, appropriateness to attend program, progression of the disease which can include conversations about care planning.
- Coordinate and manage the lunch program by delegating tasks to appropriate staff members and working with the vendor (Cinco de Mayo) on a monthly calendar.
a) Work in conjunction with Nassau County Office of the Aging by reporting in NYS Peer Place database.
- Coordinate field trips and on and off-site visits with community partners on for the day program including physical therapies, pet therapies, art therapies, etc.
- Supervise program attendance records and submit monthly billing to Director of Finance.
- Direct supervision of participants in programs, and facilitating program activities, as needed. This includes managing community partners who sponsor parties/after hour programs for day program.
- Oversee Day Program Coordinator in the management of transportation services, including but not limited to, supervision of drivers, routes, participants, maintenance, etc.
Staff Professional Development
- Coordinate and manage staff trainings, as required by NYS DOH (New York State Department of Health). This will include setting up and organizing appropriate community partners to help with the trainings, for example: CPR, fire, first aid, harassment.
- Develop a strategy and agenda for Staff Appreciation and Staff Development Days, two times a year or more.
- Implement various team building exercises through-out the year, staff recognition, employee of the quarter, etc.
- Address staff engagement levels by conducting one: on: one session monthly or quarterly based on feedback and need. Following up on any action-items with a resolution.
Engage Social Work including but not limited to;
- Responsible for taking new counseling calls, assisting new callers with resources, referrals, and program information.
- Perform clinical intake assessments, walk ins and tours, as needed, in conjunction with Director of Client Coordination.
- Perform in-home intake assessments in Nassau County for Respite Program, as needed.
- Facilitate a weekly support group and others, as needed.
- Attend administrative staff meetings, outreach activities/presentations and special events as requested by Executive Director.
- Perform other clinical and administrative tasks and projects, as designated by Senior Director of Programs and Services and Executive Director.
- Recruitment of candidates and students, in conjunction with accredited social work programs and undergraduate studies.
Other Tasks: Attend and work at LIAD fundraisers/special events 3-4 evenings per year, and attend other networking events throughout the year.
Supervise Student Interns:
a) Provide trainings, scheduling, and supervision of student interns- SIFI Certified, strongly encouraged.
b) Provide weekly one-hour supervision to each social work student, bachelors, and master's levels.
c) Discuss clinical experiences and need for growth in reading over social work student Process Recordings, and complete assessments.
d) Meetings with field liaisons via phone, email and in-person for each student twice a year, and maintain contact with field supervisors, and school field work departments.
Qualifications
- Required: MSW Degree from an accredited program and licensure; LMSW or LCSW preferred.
- Excellent clinical, organizational, oral, and written communication skills. Experience supervising staff, interns, and volunteers. Experience with the elderly population.
- Preferred: At least 3 years post MSW work, and SIFI certification (Seminar Training for Field Instruction) to supervise social work students is strongly encouraged.
Pay: $64,000.00 - $68,000.00 per year
