CLINICAL MEDICAL SOCIAL WORKER

Job Summary

We are seeking a dedicated and compassionate Clinical Medical Social Worker to join our healthcare team. Duties

• Conduct thorough psychosocial assessments to evaluate patient needs, including mental health status, family dynamics, and community resource requirements
• Develop individualized care plans that incorporate behavioral therapy, addiction counseling, end-of-life care, and other relevant interventions
• Coordinate with healthcare teams to facilitate discharge planning, home & community care, and hospice & palliative medicine services
• Provide individual, group, and family counseling sessions utilizing techniques such as motivational interviewing and cognitive behavioral therapy
• Assist patients with navigating healthcare systems, including Medicare, Medicaid, and other insurance programs; ensure compliance with HIPAA and clinical ethical guidelines
• Document medical records accurately and maintain detailed case management notes in accordance with hospital policies
• Offer crisis intervention and behavioral health support for patients experiencing acute distress or mental health emergencies

Requirements

• Master’s degree in Social Work (MSW) from an accredited program; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required
• Proven experience working with diverse populations including geriatric patients, pediatric cases, individuals with developmental disabilities or autism, and those requiring dementia or Alzheimer's care
• Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and clinical research methodologies
• Familiarity with hospital experience, inpatient/outpatient settings, nursing homes, assisted living facilities, and post-acute care environments
• Proficiency in medical documentation systems and understanding of healthcare regulations such as HIPAA and clinical ethical guidelines
• Skills in crisis management, case management, intake assessment, discharge planning, and community resource coordination
• Ability to work effectively within multidisciplinary teams practicing social work standards while demonstrating cultural competence across diverse backgrounds
• Certifications such as Basic Life Support (BLS), CPR, and experience in hospice & palliative medicine are highly desirable
• Excellent communication skills with the ability to engage empathetically with patients and families while maintaining professional boundaries

Pay: $33.22 - $40.01 per hour
