Associate Clinical Social Worker (ASW) / Licensed Clinical Social Worker (LCSW)

Angeles Community Health Center is seeking a compassionate and motivated Associate Clinical Social Worker (ASW) or Licensed Clinical Social Worker (LCSW) to join our integrated Behavioral Health team.

The Behavioral Health Provider will work collaboratively with primary care providers and other members of the healthcare team to provide behavioral health services to a diverse patient population. This position is ideal for a clinician who is passionate about community health, integrated care, and serving underserved populations.

Responsibilities

• Provide individual behavioral health assessments and psychotherapy services.
• Conduct psychosocial assessments and develop individualized treatment plans.
• Provide evidence-based interventions for patients experiencing depression, anxiety, trauma, grief, stress, adjustment disorders, and other behavioral health concerns.
• Provide crisis assessment and intervention when clinically appropriate.
• Assess patients for safety concerns, including suicidal or homicidal ideation, and follow organizational protocols for appropriate intervention and escalation.
• Maintain an active behavioral health caseload and provide appropriate follow-up.
• Collaborate closely with primary care providers as part of an integrated care model.
• Coordinate care with psychiatry, medical providers, case management, social services, and outside behavioral health resources.
• Provide patient education and connect patients with appropriate community resources.
• Complete required screenings, assessments, treatment plans, progress notes, and other clinical documentation.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Support organizational quality initiatives, including behavioral health-related UDS and quality measures.
• Participate in case conferences, clinical meetings, quality improvement activities, and interdisciplinary care planning.
• Maintain patient confidentiality and comply with HIPAA, organizational policies, and applicable behavioral health requirements.
• Perform other duties within scope of practice as needed to support patient care.

Qualifications

For ASW Candidates:

• Master's degree in Social Work (MSW) from an accredited program.
• Current and active Associate Clinical Social Worker (ASW) registration with the California Board of Behavioral Sciences (BBS).
• Must remain in good standing and meet applicable BBS requirements while working toward LCSW licensure.
• Prior behavioral health, community health, or clinical experience preferred.

For LCSW Candidates:

• Master's degree in Social Work (MSW) from an accredited program.
• Current and unrestricted California Licensed Clinical Social Worker (LCSW) license.
• Experience providing behavioral health assessments and psychotherapy services preferred.

All Candidates:

• Strong clinical assessment and documentation skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.
• Experience in an FQHC, community health center, integrated behavioral health, or outpatient setting preferred.
• Experience with an EHR/EMR system.
• Knowledge of community resources and social determinants of health.
• Ability to work with culturally and socioeconomically diverse patient populations.
• Bilingual English/Spanish highly preferred.
